Thank you for your interest in TheraPLACE. We have provided guidance on getting started below. Please contact us if would like any additional information.
At this time, we are taking appointments and do not have a wait list.
If you are interested in scheduling an evaluation, you can reach us through our Contact Us page or call us. Once your request has been made, we will match you with a therapist whose skills and availability best meet your needs. Before your evaluation can be scheduled, our Welcome Packet will need to be completed and returned. The Welcome Packet is available to download, we can mail you a copy, or you can pick one up from our office. If we do not have a therapist available during the time that you need, we will add you to a wait list and notify you as soon as the time slot becomes available.
Completing and returning the Welcome Packet below is an important step in getting started. You cannot be scheduled for services until we receive your completed Welcome Packet. You can download the packet and return it to us several ways:

How Long Will My Child Need Therapy Services?
The length of service depends on many things including a child’s strengths and struggles, family goals and concerns, consistency of attendance to therapy visits, adherence to home programming, and various other factors. There really is no answer except that it is unique to each child and their circumstances.
Do You Bill Insurance?
TheraPLACE is in-network with the following insurance carriers:
- Kentucky Medicaid
- Care Source Medicaid
- Anthem Medicaid (KY & IN)
- Indiana Medicaid
- Managed Health Services (MHS)
We understand that insurance can be intimidating and confusing. Free of charge, TheraPLACE has a Patient Advocate that can help you gain a greater understanding of your coverage details and problem solve if any issues should arise. Insurance companies differ regarding many benefit components (the need to obtain preauthorization for services, the number of visits they will cover in a given time period, and whether they require periodic re-evaluations, and others). While our Patient Advocate can help with inquires, TheraPLACE cannot render services on the assumption that charges will be paid for by the insurance company.
As an out out-of-network provider for other insurance carriers, we can provide you with a monthly, detailed billing receipt. You can submit this to your insurance company to receive reimbursement if your plan provides out of network coverage for the services provided. We suggest that you contact your insurance company to learn about your plan’s benefit details.
Do You Take EPSDT & Waivers?
We are happy to provide occupational, physical and speech therapy services under the following:
- Early Periodic Screening, Diagnosis, and Treatment Services (EPSDT)
- Michelle P. Waiver (MPW)
- Acquired Brain Injury Waiver (ABI)
If you are interested in your child being added to a waiver or if your child is currently covered under one of these waivers, contact us to move forward in getting therapy services provided.
Do I Need A Doctor’s Prescription?
Our therapists are licensed by the state of Kentucky, and a prescription is not required for evaluation or treatment. However, if you are hoping to be reimbursed by your insurance company, you will need to contact them to see if they have specific requirements.
What Forms of Payment Do You Accept?
At this time, TheraPLACE accepts, cash, checks, debit and credit cards, as well as Healthcare Spending and Flexible Spending Account cards.
